1. In consideration of the opportunity to participate and/or attend the following event , I, , do hereby release, hold harmless and forever discharge and agree not to sue ACME Inc., Landlord, Co-organizer, Host or any of the above-mentioned trustees, officers, agents, and employees (collectively “Hosts”), from any and all claims, responsibilities or liabilities for injury or damages resulting from or arising out of my attendance at, or participation in the Event Name, whether or not caused by the ordinary negligence of the Hosts.

2. I understand that this Release waiver will be used by the event holders, sponsors, and organizers of the activities that I am participating in, and that I will follow the instructions provided for said activity.

3. I certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by a qualified medical professional.

4. I consent to receive medical treatment if is deemed advisable in the event of any accident, injury, or illness during this activity. If I have a preferred medical facility, I understand I need to provide this information to the Host before this activity.

5. I understand that I may be photographed while participating in this activity.

I allow my photo or video to be used for any purpose by the host and activity holders.

I DO NOT allow my photo or video to be used for any purpose by the host and activity holders.

A. I waive, release, and discharge ACME Inc. from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this activity and/or their directors, officers, employees, volunteers, representatives, and agents, and the activity holders, sponsors, and volunteers;

B. I indemnify, hold harmless, and promise not to sue the entities or persons mentioned in this contract from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.
